Hours of operation: 

Monday-Thursday: 8am-7:30pm

Friday: 8am-6:30pm

Saturday: 8:30am-2:30pm

Upon making an appointment with Play By Day Pediatric Therapy Services we will work with you to assess your child's needs through an evaluation session.  

​

Please complete the below Intake Form prior to your appointment. Once the evaluation is complete your therapist will discuss a treatment plan and will work with you every step of the way.

​

We offer a variety of payment options and will work with your in-network insurance company to obtain authorization and approvals.

​

We are in network with the following insurance companies:

Blue Cross Blue Shield including Horizon and all out of state Blue Cross Blue Shield plans.

Cigna

Aetna

*For any other plans we will provide monthly super bills to submit to insurance for out of network benefits
